Da Vinci Vision Resource

The eye care clinic will be on-site October 9-10 from 9:00 AM – 5:00 PM at the Da Vinci high schools, 201 N. Douglas St., campus. They will be offering (ages 5 and up) convenient, no-cost access to eye exams and eyewear. In most cases, glasses can be picked up 2-4 weeks later. Each appointment is approximately 45 minutes. No walk-ins allowed. Please sign up for an appointment at this link.

Patient paperwork with guardian signature required the day of. Please see the attached forms and complete pages 3-4. Print and bring all 5 pages on the day of the appointment.
